It appears Microsoft changed their XInput driver in their latest Windows 10 Anniversary Update.

Windows 10 Anniversary Update and Xbox controller issue I also seen a suggestion of making sure your controller is connected before running uplay. I don't have time this week to test this but i thought i'd share it on the off chance it might fix it for one of you guys. I came across this from a developer fix for another game on Steam.
